Our Health Providers

Christina Dupont, Registered Dietitian #12076 with the College of Dietitians of Ontario.

Christina provides evidence-based nutrition counselling and education in alignment with the standards set by the College of Dietitians of Ontario. Her services are intended to support clients in managing and improving their health through individualized or group-based nutrition care.

Ethical Standards

As a member in good standing with her regulatory college, Christina Dupont abides by the Code of Ethics and professional standards set by the College of Dietitians of Ontario. You are welcome to request a copy of these standards at any time. If you believe an ethical violation has occurred and you have experienced harm as a result, you have the right to file a complaint with the College’s Ethics Committee.

Records

A confidential record is maintained for each client and kept for a minimum of ten years, in accordance with regulatory requirements. Your file may include your contact information and any relevant notes from individual sessions, if applicable. These records help ensure continuity of care and support the delivery of personalized nutrition services. Files are stored securely on an external hard drive, which is kept in a locked location with access restricted to the provider.

You are welcome to review your file at any time. No records will be shared with any other parties without your signed permission on a ‘Consent for Release & Exchange of Information’ form. It is your choice whether information is released and you are not required to sign any consent if you are not comfortable with it.

Confidentiality

Everything that is said via email or in the context of the conversations between service provider and client is kept confidential. There may be times when consultations may be made with another therapist or health professional. This is similar to a physician getting a “second opinion” and can be very helpful in therapeutic treatment. If consultation does occur, identifying information such as your name & surname will not be disclosed.


Limits of Confidentiality
As a Registered Dietitian, I am committed to protecting your privacy. Information you share during our sessions will remain confidential unless:

You provide written consent to share information with another health professional or third party.

I believe you are at risk of seriously harming yourself or someone else.

I suspect that a child (or vulnerable person) may be at risk of abuse or neglect, including situations involving domestic violence.

I am required to disclose information by law (e.g., via court order or subpoena).

In all other cases, your written consent is required before any information is shared..

Your Rights

As a client you have the right:

1. To ask questions at any point in time regarding therapeutic or program procedures.

2. To terminate the program at any time; you may ask me for a list of possible referral sources. (Please see the Refund Policy.)

3. To be informed of any information, decisions and actions that will affect you.

4. To ask about alternative procedures available for meeting your goals.

5. To review all documentation in your client file.

6. To complain to the Privacy Commissioner of Canada If you are not satisfied with the Privacy Policy in place.

Refund Policy

Due to the digital nature of online programs and the immediate access provided to program materials, all purchases for The Blood Sugar Reset and other digital services are non-refundable.

I am committed to delivering a high-quality experience and supporting every participant through the process. If you are unsure whether this program is the right fit, I encourage you to book a complimentary discovery call prior to enrolling.

Should unforeseen circumstances arise, accommodations may be considered on a case-by-case basis and must be requested in writing. Please note that refunds are not guaranteed and are not issued for missed sessions, non-participation, or partial use of the program.

Program Length & Fees

Fees for this program are due in full or in part according to our payment plan. This includes access to all program materials, live group sessions, replays & recordings, resources, and direct support from the dietitian throughout the 12 weeks. Payment can be made via credit card only.

Informed Consent

By clicking below, I agree that I have read and understand the above information, and agree to the terms of service stated above. My Service Provider has adequately answered any questions I have at this time (via email).

I understand that I have the right to withdraw from the program at any time and may request a list of referral sources if needed. I also understand that it is generally best for both the Service Provider and client to make a joint decision regarding the conclusion of services.

My signature indicates that I am giving my consent for Christina Dupont, Registered Dietitian and Certified Diabetes Educator, to provide nutrition counselling and support as part of The Blood Sugar Reset group program. I acknowledge that I may save a copy of this agreement for my records..
